How to apply as a Provisionally Registered Pharmacist (PRP) in School of Pharmacy, Monash University Malaysia?

REQUIREMENTS

Candidate must:
  • be a Malaysian
  • have completed BPharm degree in Malaysia that is approved by the Pharmacy Board of Malaysia.
  • currently enrolled as a graduate research student at Monash University Malaysia and must have successfully completed the confirmation of candidature (1st year milestone).
  • have an assigned research project related to pharmacy and pharmaceutical science.
  • apply for their own PRP Registration number in PRiSMA
  • apply before Mid-Candidature Review Milestone (2nd year milestone).
  • obtain approval from the main PhD Supervisor.

APPLICATION FOR PRP IN ACADEMIA

  1. Express interest in pursuing PRP R&D Academia in School of Pharmacy, Monash University Malaysia
    a. Inform Dr. Charmaine Teoh (PRP Coordinator), Dr. How Chee Wun (Graduate Research Coordinator), and Ms. Siti Intan Rohayu (Education Management Office)
  2. Identify a Registered PRP Preceptors (Item 9) at the School of Pharmacy (SOP, MUM).
  3. Identify the date for commencement (after discussion with the selected preceptor).
  4. Prepare the required documents for registration with PBM.
    a. Graduate Research Offer letter from Monash University
    b. Signed Indemnity Form
    c. Research proposal
    d. Complete FLL (ii) Form
    e. Offer letter for PRP in Academia from SOP, MUM
    f. Signed Monash SOP-PRP Agreement
  5. Email all the documents to the Pharmacy Board of Malaysia at latihan.prp@moh.gov.my. Carbon copy (email cc) to the Preceptor, Dr. Charmaine Teoh, Dr. How Chee Wun and Ms. Siti Intan Rohayu at least FOUR (4) weeks before the date of training stated in the Monash SOP-PRP agreement.
  6. Begin PRP training upon receiving approval from PBM. Keep record of the daily research activities in the logbook prepared by PRP (refer to more information in the logbook).
  7. 4 months after PRP training commencement, identify the date for hospital attachment (must be completed within 12 months of PRP training). Inform Dr. Charmaine Teoh to fill the online form (which will be provided later).
  8. Here is the list of the training hospitals.
  9. Upon approval (notification letter from training site), apply for intermission.
  10. Submit the following documents to Dr. Charmaine Teoh, Dr. How Chee Wun and Ms. Siti Intan Rohayu.
    a.  A copy of notification letter from the training site
    b.  All documents in (4)
    c.  Signed Page 2 of PRP Academic Logbook
  11. Upon completion of PRP training, obtain PRP preceptor’s endorsement to PRP Academic Logbook
  12. Submit the completed PRP Academic Logbook to PBM via email and cc the Preceptor, Dr. Charmaine Teoh, Dr. How Chee Wun, and Ms. Siti Intan Rohayu.
